We will not retaliate against you for filing a complaint.


5. Our Responsibilities

  • We are required by law to maintain the privacy of your PHI.
  • We will notify you if there is a breach involving your unsecured PHI.
  • We will follow the terms of this Notice and update it as required by law or policy changes.
